I am paying with check, cash, or money order (checks payable to AAISDA, Inc). I will leave the payment at the school. I would like to make my payment through Zelle. Payee is: [email protected] I am making a one-time payment with a credit card and will add $2 to total (convenience fee). In order to make future payments I will notify AAISDA, Inc of payment method (credit card, check, cash, money order). I would like to make payments EVERY month with my credit card I understand that the card will be charged on the 1st of the month. There will be an additional $2 (convenience fee) added to the tuition amount each month. I understand that a TWO WEEK written notice must be given in order to stop auto pay. If insufficient notice is given, and the credit card is processed, NO REFUNDS will be given. If the charge is reversed by the cardholder a $25 fee will be added. Your credit card statement will be processed by Arizona American Institute for Self Defense Arts.
